You must bring with you:
• Photo ID & Social Security/Tax ID cards for everyone being claimed
• W2 and/or 1099 Statements
• If claiming child care expenses, bring documentation and care provider Tax ID number
• Last year’s income tax return
• If you received unemployment insurance in 2009, bring Form 1099-INT
• If self-employed, bring a self-declaration letter stating your income in 2009 and receipts/ledger stating your business expenses
• Any relevant tax information
